Provides short term client centered case management from an integrated model of care. This includes, information and education around harm reduction (which includes abstinence), substance use, distribution of harm reduction equipment, advocacy, care coordination, referrals to appropriate programs/services and groups, such as relapse prevention.

The Harm Reduction Counselling and Supports team supports individuals to reduce any potential harms related to substance use. The program serves people 25 and older who use drugs and who can not afford to pay for services. Counselling services and outreach support, is provided, where possible.

We offer FREE Detox and Withdrawal Management for individuals suffering from substance use concerns in the catchment areas. We provide an addictions physician, nurse, relapse prevention worker as well as a counsellor. Six months of relapse prevention, and then 10 free counselling sessions. All services are offered free of charge. Services are also offered in English, Punjabi, Hindi and Urdu.

Based out of the Habitat at 81st Building, entrance off of King George Blvd Surrey BC. But also offering services to Surrey, Langley, Cloverdale, White Rock, Ladner, Tsawwassen, Delta.

This program provides short-term counselling to individuals who have been discharged from the hospital emergency department and are at risk to re-enter the hospital emergency department for mental health and addictions issues.
